The very best advice I can give you is to simply start writing. Sounds easy huh? Really, forget about all the notes for right now. Forget the chapter headings and all of that, since you have ADHD, just write from the gut until you have nothing left to say! Don't worry about how many pages or any of that, simply write, write, write... and then please get back with me! Start right now :-) Talking and writing about your book will help, so feel free to blog about your experiences here. Your blog posts could become part of your book, or, even turn into chapters!

I think it's great that you want to write a book about the great milestones of life from the ADHD perspective. Many will be interested in such a book.
Have you thought of self-publishing? It is often a good way for a first-time author to go. The large publishing houses seem to focus on established, best-selling writers. For this reason, one option might be to try www.lulu.com, a site for self-publishers. You might just visit the site to see how you can calculate the price of a book with certain quantity orders (very small orders, too). Just a thought.
Also, you might think about carrying a small, hand-held mini-recorder in your pocket for when you get an idea. Then you can write it down later at the computer when you have gathered a list of ideas on the recorder.
