I hate doctor's office questionairres. There is never enough room to explain why you checked certain spaces. I usually have to draw arrows to the back of the page to explain why I checked both the "yes" and "no" boxes.

- About me & Relation to ADHD or other condition?
- I'm a divorced mom of a 10 yr old son with ADHD. I strongly suspect my former husband / son's dad may have ADHD, too. I think I've been living with it, too. I'm going to get tested on Dec. 15th.

ADHD Evaluation - Not so fast!
I went to a 3-hour appointment yesterday. It wasn't what I expected. Turned out to be an introduction for all new patients.
Upon my 8:30am arrival, I immediately filled out registration paperwork. The clipboard of pages I had spaces to fill in basic personal info including a questionnaire. One of the questions was "Have you ever used alcohol." Ever?! Well, yes...I'm almost 40 now and in my 20's I attended my share of happy hours...especially after my first divorce. - Well it's certainly hard to tell if you have it or not. You haven't said much, but what you have said has been very focused and central to the main point. I'm not sure really. Curious to see the results.
